Instruction:
- Print the 1st page(1 copy only, do not fill up).
- Print the 2nd and the 3rd page into 1 copy back-to-back and fill up. Produce 3 copies.
- Bring the printed 1st page and 3 filled-up forms to the Business One-Stop Shop at City Hall for processing.

All Business are required to secure a Business License and Mayor’s Permit prior to operate a new business or to continue operating an existing business. To facilitate the renewal of business permits, at One-stop-Shop located at 2nd floor, City Hall. If requirements are complete, the transaction can normally be completed in one (1) working day.
WHO MUST AVAIL OF THE SERVICE:
Citizens doing business or plan to do business within the city.
BUSINESS PERMIT REQUIREMENTS:
- City Economic Enterprise Office – Market Clearance ( Stall Holders only)
- Barangay Clearance – Barangay Business Clearance
- Planning and Development Office – Zoning Clearance
- City Health Office – Sanitary/Health Permit
- City Engineer – Certificate of Occupancy (NEW) / Certificate Annual Safety Inspection (Renewal)
- ENRO – ENRO Clearance
- City Treasurer’s Office – CTO Clearance Fees/Bus. Tax/RPT
- DTI – Business Name Cert. of Registration
- SSS Clearance
- PhilHealth Clearance
- Pag-ibig Clearance
- BIR (R-VAT) - Clearance
- City Fire Department – Fire Safety Inspection Certificate
- City Vet. Office – Pest Control Clearance
- PNP – Police Endorsement (for Billiard and Mahjong)
- Optical Media Board – Certificate of Accreditation (as maybe applicable)

All businesses are required to secure a Business License and Mayor’s Permit prior to operate a new business or to continue operating an existing business. To facilitate the renewal of business permits, one-stop-shop located at 2nd Floor, City Hall. If requirements are complete, this transaction can normally be completed in one (1) working day.

REQUIREMENTS:
- Tax Clearance from city Treasurer’s Office. Business Permit/Tax Clearance for New or Renewal of Business Permit
- Environmental Permit from City Environment and Natural Resources Office (ENRO)
- Barangay Clearance (site of business)
- Certificate of Annual Safety Inspection from City Engineer’s Office. Certificate of Annual Safety Inspection (CASI) (Public Market vendors are exempted)
- Food Handler’s Certificate or business engaged in food service and/processing. Issuance of Food Handler’s Certificate and/or Sanitary Permit
- Clearance from Economic Enterprise Office (Public Market) for market vendors. Issuance of Market Clearance for the Renewal of Business Permits and Licenses.
- Zoning Clearance from City Planning and Development Office. Preliminary Zoning Clearance
- Certificate of Fire Safety Inspection from Bureau of Fire Protection
- Department of Trade and Industry Clearance
- SSS Clearance
- BIR Clearance
- Phil Health Clearance
- Pag-Ibig Clearance
- Duly Accomplished and Notarized Mayor’s Permit Application Form
