Certified true copies of Civil Registry records are issued upon written request.

REGISTRATION OF BIRTH

A child has the right to a name and identification and should be registered within 30 days after birth as evidenced by the Certificate of Live Birth (COLB). Beyond this period, the Local Civil Registrar will issue a delayed registration of live birth to ensure that the child’s right is protected.

If requirements are complete, this transaction can normally be completed in eleven (11) succeeding days. On-time registration could even be shorter.

  2. Attendant-at-birth or hospital liaison officer if the child is born in a maternity clinic/hospital.

APPLICATION FOR MARRIAGE LICENSE

A marriage license is required before the solemnization of marriage. Normally, this transaction can be completed in twelve (12) working days.

  4. One or both applicants are residents of Dumaguete.

REGISTRATION OF MARRIAGE CERTIFICATE

Marriage is a special contract of permanent union between a man and a woman entered into in accordance with law for the establishment of a conjugal and family life. It is the foundation of the family and an inviolable social institution whose nature, consequences, and incidents are governed by law, and not subject to stipulation.

In ordinary marriage, the time for submission of the Certificate of Marriage (COM) is within  fifteen (15) days following the solemnization of marriage while in a marriage exempt from the license requirements, the prescribed period is thirty (30) days, at the LCRO where the marriage was solemnized.

  5. Notarized affidavit of delayed registration executed by the solemnizing office stating the exact place and date of marriage, the facts and circumstances surrounding the marriage and the reason or cause of the delay.
  6. Certification from the LCRO of non availability of record and CENOMAR of both parties from Philippine Statistics Authority (PSA) (formely NSO).
  7. If requirement No. 1 and 4 are not available, a Certification from the solemnizing officer stating that no record is available in his office file and a Joint Affidavit executed by both contracting parties with a corroboration from two (2) witnesses stating among others: HUSBAND’s and WIFE’S full name; Date of Birth, Age (at time of marriage); Place of Birth; Sex; Citizenship; Residence; Civil Status; Religion; Name of Father and Nationality; Name of Mother and Nationality (maiden name); Date and Time and Place of Marriage; Name and Title of Solemnizing Officer; Name of Witnesses and Names of Children supported by each and certified copies of the birth certificates.

FILING OF PETITION FOR CHANGE OF FIRST NAME (CFN) OR CORRECTION OF CLERICAL ERRORS (CCE) UNDER R.A. 9048 AND R.A. 10172

R.A. 9048 authorizes the Local Civil Registrar and the Consular Offices to accept petitions for corrections of clerical errors and change of first name in the Civil Registry record, and expanded by R.A 10172 Correction of Child’s Sex and date of birth (month and day only). Normally, this transaction can be completed in two (2) to three (3) months.
